because if a doctor or somewhat with the fortitude to be one can get by with the same rewards of being a janitor, why would they push themselves? there has to be an initative.. humans aren't that altrustic to think it would happen otherwise

But they forget that even a society where everyone gets food/water/tools etc
needs people picking up trash and poop

You've got to consider the reasoning behind different career choices. Some people arrive at the same career through different paths and personality types.
