Also VA doesnt cover dental, and obviously the psych treatment has issues, they hand out antidepressents like candy

Most fat people you see in tv shows, or movies usually wear fat suits. For example John penet was on hairspray and he had to lose over 100 lbs then wear a fat suit. Also mr Dursley from Harry Potter used a fat suit.
