Self image can be literally anything you see yourself as.
If I see myself as a nice person, the very definition of generosity, that doesn't matter if I actively tell every homeless person I see to ' get a fucking job '
I, personally, see myself as being a bit younger than I am. In my head I'm about 23, I imagine myself to have a different jawline, a certain intense defiance when it comes to matters in questionable authority, and a strong philosophical grasp.
There are things in that self image that, to be healthy, I have to accept as untrue. Other things that I have to actualize if I want to be perceived that way.
But if someone does not see me that way, if they see me as ignorant, or just plain arrogant, that is their image of me, no matter how I disagree. They're not obligated to tell me how smart I am or grounded I am, or how righteous I am in the fights I take up, that would be silly. Nobody's obligated to call me stunning and brave.
Cool, so if you imagine yourself to be a woman, but I believe you're a man, you aren't entitled to make me recognize your imagination.

It's not a negative state.
Intersex provides evidence of how the brain knows gender. We've seen intersex kids or kids with circumcision accidents where physicians and parents try to assign. The kid grows up with anatomy that sexes them but they know it's wrong. The brain knows.
The brain doesn't innately know gender.
