Well, Milo Yiannopoulos tried to use the term to label conservatives that were tired of the GOP and partisanship, but Spencer and friends were already using it to label themselves.
The earliest usage seems to be from 10 years ago.

I've heard Milo used the term before he knew Spencer was using it.
So it's a bit confusing.
I saw that, in his cathy newman interview, where he said "A fresh new movement for conservatives with new ideas"
Later Milo gave up on trying to define the term himself, and just let Spencer have it.
Now he just refers to it as him being a bit misinformed about the usage of the term.
CNN framing Spencer as the leader of the Alt-right didn't help
But, there are black people on quora who also claim to be alt right
And i'm confused
It's a bit like the Skeptic™ Community... it's not well defined, in the first place.
I don't think it's fair to say it "started as...", because the root of the term, "alternative right" fit more than one group.

The change didn't happen within a single group, it's just one group kept using the term, the others decided to abandon it.
