Taxation and capital flight, if you want to make a free-at-the-point-of-service healthcare system (like what MOST OF THE WORLD HAS) you have to tax your citizens. Those with large sums of taxable money (those needed to make these programs functional) will simply move states because they can. This is why tax havens exist, and how the rich avoid a LOT of taxes.
I can't see why you should be more compassionate to someone just because they were born in the same country.
