Because thats the insurance model. Insurance is a just in case to help pay, not meant for your everyday. Look aat car insurance. It doesnt provide repairs it helps in the unfortunate but also doesnt pay for routine maint.

There are many factors to that. Creation of the fda, america increasing its r&d rates, various insurance regulations, ect.
