People economise, even when their life is threatened. Dosen't matter if it's anecdotal. Everyone is a decision maker. If you would call an ambulance, you clearly see it as worth it in that instance. If the service was provided for "free", you would obviously pick a "free" ambulance.

Whatever the state count as "healthcare" is covered by health insurance plans, allowing them to essentially adjust the price of insurance premiums whenever they want, by artificially restricting supply.
