So let's say someone has a problem with their heart or another vital organ and needs surgery to fix it and improve their chances for survival. Or even if they have depression and need medication to help them find relief from it. Is it immoral to go through with the surgery or take the medication because you're rejecting the role you've been given, i.e. being unhealthy and miserable?

Well, I do know that people of certain religious affiliations (I think Jehovah's Witnesses do this?) refuse to accept organ transplants because they think it's immoral

Anyway, how do we tell which things are illnesses and which are just expected roles? If your "expected role" is causing you distress (i.e. gender dysphoria), would it not make sense to treat it as a form of illness as well and try to mitigate it by changing the thing that's causing you distress in the first place? It seems to me that your argument could just as easily be applied to, for example, depressed people, as it could be said that being depressed is just their given role designated by their biology and that it's immoral to try to change that.
