Easiest way I could think of, is if you tried to feed an extra round through the ejection port

I've never seen this happen before. I looked online and most the pics are just of a spent brass case getting in there, which makes more sense. Case fails to eject and the next round pushes the case up, getting it jammed when the bolt carrier goes back forward.

The remedy is to try and push the bolt back with a finger and pull the charging handle back and forth until the case falls out
