A nation should care of non-citizens in domestic affairs because:
-Hippocratean oath. Doctors have to treat all patients that require treatment. If a healthcare service is not provided that doesnt include foreign citizens, doctors cannot follow their oaths. Doctors not following their oaths sets a predicament that they do not, in all cases, have to treat patients. A slippery slope in other words.
- Law enforcement. Foreign citizens can commit crimes, that from varying degrees require different enforcement of law. If a penalty for crime X requires incarceration within the country, the prisoner has to be placed within that nations prison system. A law enforcement that "doesn't care" of non-citizens commiting crime is a dysfunctional law enforcement.

@The Desert Fox V 1. that would still be in breach of the Hippacratean oath, that in fact makes a doctor a doctor, not just someone who can put bandages on. There are loopholes like you'd have to isolate the country from any foreigners, but that would then bar any diplomacy or embassies.
