Generally try to shoot the people shooting at you, and usually try to avoid getting into those kinds of scenarios. There are a huge amount of ways to try and minimize risk.
Civilians don't start or participate in wars - especially those in their own neighborhood. I am fairly confident that no American serviceman ever wants to be responsible for civilian deaths regardless of circumstances. I think there are people in this channel right now that can attest to this with firsthand experience. They can chime in, if they wish.

Innocent people die all the time in war, terrorists don't give two shits if civilians are in the way because they don't value those lives. They will attack you any place any time of the day.
Most cops manage to not kill over a dozen civilians when responding to an active shooter situation
