sight distances are irrelevant because the distances are so great that you can't control everything between the 2 points. But even if you ignore that, you still see things being hidden from the bottom up

For viruses like the flu, yeah, just certain strains. But for, say, Measles, the strains don't change enough from one another to be unaffected by the vaccine.
