The symbolism is said to be the blood of the martyrs. Realistically, that’s a symbolism that has been read back into the tradition. The pope wears red shoes because he was (and is) usually a cardinal before being elected. The shoes are those that the cardinals wore in olden days - red shoes and a red cassock or simar.
