Had this happen few times with old ones and you can get seriously hurt probably
anyways its annoying
yeah that is dangerous
you need to either set your handle by hitting the top of it so it kinda splays out over the actual ax-head and keeps it in place
or if it's too far gone just get a new handle
the latter of which though isn't something I've had to do yet

Use the ring-style wedge instead of the flat plate.
Seat the handle 1/8" below the top of the axe head, coat the ring wedge in epoxy, seat it, then fill the 1/8' area with epoxy.
