When I was younger he worked with people with traumatic brain injuries, and old folks who'd had strokes.

A lot of what he did was what are called swallow evaluations.
That is when someone had a stroke they would call him in (after recovery, etc).
He would administer a battery of liquids and foods starting with water and moving in thickness.
Kinda like the Mohs hardness scale, but for food.
He would feed them progressively more and more solid foods until they choked.
So a lot of his work was basically getting as close as you could to choking someone to death without actually doing it.
