So far it seems that you've mostly passive about giving him your advice, and maybe even ending it with "but that's just my opinion tho". Just be real with him. Next time you see him just say it as is and that while you do appreciate your friendship, the one-sidedness of it is getting tedious. Be honest and tell him that you're getting sick of constantly giving him advice he never listens to, and that if he wants you to keep on being his friend be better get his shit together. Make it as clear as day that this is indeed an ultimatum and that if he doesn't get his shit together, you're done.
