Its a shell game, a doctor can provide medicare coverage at cost + small margin. Then the paying customers pay for all the administrative overhead

Another topic often skipped is that 77% of medicare users combine medicare with a supplement plan. So even if there was a medicare for all, almost everyone would still be paying for a private insurance to supplement it. Which is also what happens in Canada and the UKs "free healthcare" schemes
