I don’t particularly understand why they shouldn’t seek psychiatric counseling to alleviate their dysphoria and avert the need to undergo a pointless surgery that’s essentially otherwise equivalent to medical mutilation.

This concern is less focused on realistic psychological well-being of the individual and allowing them to accept who they are, as determined by their chromosomes, rather making it a ham-fisted attempt at achieving their “happiness” in the clumsy way.
