Usually doctors just assign you a gender and perform the operation right there without ever telling the parents or the baby when they're older.

I have a friend that happened to.

Calling someone the opposite sex is considered to be insulting in most cases, because male and females strive to be males and females respectively, so if you call them the opposite thing, that means they exhibit a different behaviour from what people of their sex would normaly do. The vast majority of people strive to act according to their sex.
