From what me and him could figure is this happens, its called a squib. It can happen with factory ammo, but it was probably someone who reloads their own ammo Either the projectiles too big for the gun their using or they didnt put enough powder in it to get it all the way out. It'll get stuck somewhere in the barrel, then like Saint said, the shooter dosent realize the bullet didnt exit and shoots again

I have one guy in Arizona and one guy in Nevada That loads my 75 grain 223 and 5.56 ammo and these guys are spectacular in case anybody wants any specialty ammo
