A lot of the times we correct ourselves and send out the lesson learned voluntarily

It's kinda fun actually, people get creative describing the mistake, make jokes, but overall keep it informative.

We never have to send out the lesson learned or anything like that we just fix it and move on. We don’t bring it up we just grow.

We also have all officer meetings, AOMs where we discuss safety and training in an informal setting, usually with beers in the wardroom
