Would this be an adequate template for the new script?

No, the new template is designed to eliminate the separation of the intro questions to the rest of the notes. After the Interviewee ID, Age/State/City, Conductor, and Notetaker, the ONLY thing that should follow are Q1: Answer, Q2: Answer, and so on. Don't make it Q1: Age:, just make it Q1: 25 years old. Additionally, take out the sections highlighting the difference between Filtering, Evaluation, etc. Those are ONLY for our organization in the script and should not be recorded in the Notes.
