The arrow piercing his body, his toned, effeminate but overtly masculine appearance, and the way it juxtaposes the depictions of other saints as charitable. The translated phrase of the character's reaction is that it illicit 'pagan joy', but he also notes that it reminds him of the warrior culture of literal pagans as opposed to the favorable depiction of Christian saints. It's probably intended to illicit shock, the painting, and to make the viewer see the pagans as savages.
Why did the MC jerk off to it? He was twelve and his mother kept him away from boys for most of his childhood, combined with him being sickly and constantly off school.
So, perhaps his submissive nature, and his lack of opportunity to partake in masculine behaviours, encouraged his homosexuality. It was something his parents worried about, his mother was appalled by his cross-dressing.
