The template is available at most office supply stores, and online. Buy the paper, and then print this pdf on it. Designs should line up with the labels, and you simply cut them out from that.

What lines?

The two thick horizontal lines are supposed to not be centered.

That's where the name of the org goes.

The one on the right side is higher than the left

Both of the lines extend to the right side.

No, I mean on the pdf, the sticker on the right side has its lines slightly higher than the left side sticker. It's uneven.
