I immediately hold him upright once I hear the poop, and unzip/unsnap his outfit from the bottom, pull it up to his back (check for poop), then transfer him to the changing pad back first while instantly grabbing his legs upwards so he never puts pressure on his bottom, which would force the poop up and out of the diaper.
